What To Do If You Have a Problem

Step 1: Talk to a member of your health care team

The best place to ask questions, get information or make suggestions is your dialysis or transplant facility. We encourage you to talk with the members of your health care team about your concerns before they become problems.

Step 2: File a grievance with your facility

Every facility is required to have a grievance procedure prominently displayed in the patient waiting area. Follow the instructions for filing a grievance.

Step 3: Call the End Stage Renal Disease Network of New York, the NY State Department of Health, or the Centers for Medicare & Medicaid Services

If you have a problem with your dialysis or transplant facility that has not been resolved, or if you are concerned about talking with your health care team, the End Stage Renal Disease Network of New York, the Centers for Medicare & Medicaid Services or the NY State Department of Health can assist you to file a complaint. See the list below for the office nearest you.

Remember: Not everything that is a problem for you can be changed or fixed. Even if something can be changed or fixed, it may take time.
