To report an operational status change (open/closed/altered schedule) to the Network, please complete the attached Emergency Operational Status Report Form or scan the QR Code below. It is equally important that your local Department of Health be informed of any changes in operational status. If you need assistance from the Network, be sure to indicate so in the report.
For emergent and non-emergent events impacting dialysis facilities, complete and submit the form below.
