To report an operational status change (open/closed/altered schedule) to the Network, please use one of the links below to access the appropriate report. It is equally important that your local Department of Health be informed of any changes in operational status. If you need assistance from the Network, be sure to indicate so in the report.

  Emergency Incident Reports

Emergency Incident Reports must be completed to document any altered schedules or clinic closures that are not being reported in EQRS. This includes, but is not limited to:

  • Staffing shortages
  • Supply chain disruptions
  • Facility or infrastructure damage
  • Natural disasters
  • Cybersecurity events

In the event of a widespread emergency (e.g., hurricane, snowstorm, tornado), your facility may be asked to report operational status in the EQRS Emergency Reporting Module. If selected, your facility will receive notification via email from KCER and is only required to report this event in EQRS.

  Permanent and Non-Emergency Closures

Non-Emergency Closures must be completed to document any permanent or temporary closures. This includes but is not limited to:

• Permanent closures
• Long-term construction
• Clinic relocating
• Closure for over 30 days
