All Medicare-certified dialysis facilities, as well as renal transplant and Veterans Administration (VA) facilities, can submit ESRD program administrative and clinical data to CMS through the End Stage Renal Disease Quality Reporting System (EQRS).
About EQRS
The End Stage Renal Disease Quality Reporting System (EQRS) is a web-based data collection system mandated by the Centers for Medicare and Medicaid Centers (CMS) to enable dialysis facilities to meet portions of the Conditions for Coverage (CfC) for ESRD dialysis facilities. EQRS helps the renal community transition from paper-based data collection to an electronic "always-on" format. The system is designed to help improve patient care efforts by reducing the time it takes CMS to produce clinical performance results. EQRS functions as an important data source for the ESRD Quality Incentive Program (QIP) performance measures. ESRD Networks and dialysis facilities use EQRS to enter and submit patient and clinical quality of care data to CMS.
